Rural Health Network is a Federally Qualified Health Center (FQHC), which means we can offer a variety of services via a sliding fee scale. As an FQHC, Rural Health Network accepts Medicaid and Medicare insurance plans, in addition to many other insurances.
The sliding fee scale is designed to help meet the needs of uninsured and underinsured patients. It provides reduced costs for many services to those who qualify. No patient will be denied access to services due to insurance status or ability to pay.
Sliding fee scale services are offered to eligible patients, who provide documentation of annual household income. Income guidelines and acceptable verification for sliding fee scale include but are not limited to the items listed on below “sample” chart.
Documentation to verify annual household income can include (but is not limited to): tax returns, W-2 Forms, two (2) most recent payroll stubs, social security benefit statements, retirement plan benefit statements and attestations by persons making payments to household members and/or other documents.
